We have a very flexible process to set up services. Our goal is to meet you where you are at and set up a service that works for you.

We will require two documents be completed prior to getting started:

1. Service Agreement – this is an agreement between the client / Guardian, Trustee / funder/ Power of Attorney and Plan It Consulting. This document gives us parameters for providing the service. You are only billed for services rendered, not services outlined in the contract.

2. Client Information Sheet – This is specific information about the client that we will need to provide the service. We rely on this information for the matching process of the suitable companion. Complete the information that you feel we will need to provide a quality service.

We would be happy to meet with you to complete the paperwork.

Matching you with a suitable companion(s) can be done before or after the information is completed although having the information ahead of time is beneficial to ensure a successful match. Please let us know if you would like to be involved in the matching process.
